I know that both of you are reluctant to ask for help, but I'm also guessing that your trip to Japan is going to be hard on Pat, given that she is not feeling well, her energy level is low, and how recently you got back from Spain.
I'm wondering if there are ways that folks could help you out-- bring the boys to W&S so Pat could have an evening to herself, take the younger two out of the house for a while this weekend or next, make a casserole, etc. (my weekend includes things like Allegra's school festival, Mark's choir concert, and a kid-friendly sukkah-building party, and I could take them for a bit, and perhaps team up with other friends so that one of us drives over, has them for a while, the other one has them for longer and returns them home.)
